The Verdica score is a composite trust score from 300 to 850, computed from certification status, quality measures, and inspection reports maintained by CMS. A score of 850 places CANTERBURY NURSING AND REHABILITATION CENTER in IN in the excellent (800–850) band. The score reflects publicly available regulator data — it is not a rating from Verdica itself and does not incorporate paid reviews or advertising.
Verdica combines three weighted sub-scores drawn from CMS. Each sub-score reflects a different dimension of healthcare compliance — CMS star ratings, deficiency findings, and complaint outcomes — and is normalized against industry peers before being aggregated to the 300–850 scale.
Verdica pulls public records from CMS: certification status, quality measures, and inspection reports. Data is refreshed on rolling cycles, typically weekly to monthly depending on the source agency's own update schedule. See the "source data current" indicator on the entity page for the last successful refresh.
Common negative signals include immediate-jeopardy findings, high deficiency counts, or termination notices. Any of these appear in the underlying regulator records and pull the sub-score down; the composite reflects the weighted impact.

I arrived here yesterday after being misled that it is now a new place called eagle rock subacute rehab and was upgraded. I arrived at approximately 4:30 pm from RWJ st barnabus and the room assigned was filthy, sparse and absent of needed hygiene products (pads for bladder accidents). I was not assesed until 9:40 pm and then very cursory. My medication was not ordered and and my pain meds after a hip replacement were not available. Nobody has assisted me with sponge bathing and i do not feel safe here i am not paranoid . I was marketed to come here and misled having been told they have ‘changed things’ and are upgrading. I have experienced about 5 aides and 2 nurses ( it is now 5:15 am ) and only one has been adequate. I am very upset and have contacted cloverhealth for a transfer. This facility does not feel safe.

I would strongly urge families to think carefully before placing a loved one with Alzheimer's disease or memory-care needs in this facility. My father, who suffered from Alzheimer's, spent his final days here, and the experience was heartbreaking. The memory care unit felt outdated, dirty, and neglected. The environment was depressing and institutional, more like a prison than a place where vulnerable seniors should spend their days. The rooms were often unclean, and there were many occasions when it appeared that basic hygiene and housekeeping were not being properly maintained. One of my biggest concerns was the level of care provided to residents. My father often appeared overmedicated and less engaged than he had been before arriving at the facility. As a family member, it was difficult to watch and even harder to get clear answers about his care. The most devastating part of our experience began when I received a call informing me that my father had fallen after losing his balance. Three days later, he suffered a stroke, and he passed away the following week. Returning to that facility to spend my father's final days with him is a memory that will stay with me forever. What makes this even more painful is that several staff members privately approached our family and expressed concerns that something seemed wrong with my father before the stroke. We were told that these concerns had been communicated to the night staff but were allegedly not acted upon. Hearing this after his death was devastating and left us with many unanswered questions. To make matters worse, while my father was literally on his deathbed, the business office continued to contact and pressure our family regarding administrative and financial matters. At a time when compassion should have been the priority, we felt harassed and ignored. No family should have to wonder whether their loved one is being properly cared for during the most vulnerable period of their life. My father deserved dignity, attention, and compassionate care. Sadly, that was not our experience. Based on what my family witnessed, I cannot recommend this facility to anyone seeking memory care or Alzheimer's care for a loved one.
The receptionist is a rude and unprofessional person. This place needs a better staff crew.
Overall bad service. No compassion for patients. Awful communication and nurses never have answers. No doctors onsite.
receptionist is rude and not helpful. Needs better staff training
PT OT were excellent. After that the nursing and aide staff were non attentive, disrespectful, and outright negligent. If your loved one is incontinent, stay away.
I went to this place yesterday, The first floor of the office smelled good and it was nice looking But the minute you went to the unit with patients Who has Alzheimer’s/psych, I walked in and out of the unit! It was so bad and the whole entire unit smell like piss, There was no structure And scary. I felt like I was in a movie. I don’t understand how the main lobby is more updated than where our patients are at. I do not recommend this place if you love your family member And I do not recommend this place for any Alzheimer’s patient care. The room was disgusting And that place was dark, There is no hope. They have A new management, So be careful with that! Please make sure that you go to these nursing homes And make sure your loved ones are taking care of and the places is up to date, Praying over this facility, but it should be closed down. I love my mother too much to put her in a place where it’s not healthy at all or safe!
I would like to say that this place did have a huge impact on me getting better . The nurse Zoey was great . Anna at admissions was amazing and definitely put my family at ease . The two counselors are great . Definitely made sure to get me everything I needed to make sure my transition to back home was easier . Especially when I had an issues about an hour before leaving there she personally went out her way and drove to meet my doctor to make sure everything was handled ! My OT and PT were the best !!! Made sure I got that everyday even when I didn’t want to get up . .. I would have to say a few bad things and I can’t not say it .. the director or head of nursing whatever she was on the bottom floor when you walk in to the right was terrible . She was nasty rude and made me and my family completely uncomfortable for how she treated us .. even made my small child cry . . I didn’t see her for my whole stay until 6 days out the door almost and she was just awful . It’s not to say and it’s pretty obvious that the place needs to be redone . It looks like it hasn’t be touched from 1956 .. but they did get new owners and one of them made sure my first week in to see if there was anything they could change etc .. one thing I can say the food doesn’t even Look edible : I had to order all my meals .. otherwise I wouldn’t eat . So 4 stars for all the good things and good people in there .
