The Verdica score is a composite trust score from 300 to 850, computed from certification status, quality measures, and inspection reports maintained by CMS. A score of 850 places MIDDLETOWN NURSING AND REHABILITATION CENTER in IN in the excellent (800–850) band. The score reflects publicly available regulator data — it is not a rating from Verdica itself and does not incorporate paid reviews or advertising.
Verdica combines three weighted sub-scores drawn from CMS. Each sub-score reflects a different dimension of healthcare compliance — CMS star ratings, deficiency findings, and complaint outcomes — and is normalized against industry peers before being aggregated to the 300–850 scale.
Verdica pulls public records from CMS: certification status, quality measures, and inspection reports. Data is refreshed on rolling cycles, typically weekly to monthly depending on the source agency's own update schedule. See the "source data current" indicator on the entity page for the last successful refresh.
Common negative signals include immediate-jeopardy findings, high deficiency counts, or termination notices. Any of these appear in the underlying regulator records and pull the sub-score down; the composite reflects the weighted impact.

The compassion, dignity and exceptional care received for my very dear friend of 28 years was everything I hoped for. Every member of the staff and management treated her with kindness, and respect while supporting me during this extremely difficult time was beyond measured. They made a heartbreaking experience as peaceful and comforting as possible. I will be forever grateful for their professionalism, empathy and unwavering dedication. I will never forget everything you did.

Mom is in for rehab for ALS. So far so good. Friendly and clean.
Our family will be forever grateful to Middletown Park. My father spent his final 100 days here, and every day he was treated with dignity, patience, kindness, and professionalism. Years ago, my dad served here as a Eucharistic minister. When he reached his own time of need, there was nowhere else we wanted him to be—and that trust was honored. From admissions through long-term care, the staff consistently went above and beyond, doing for him what our family could not, always with respect and compassion. When my father declined, my sister and I were welcomed to stay bedside 24/7 until he passed peacefully, surrounded by family and staff who were gentle, loving, and attentive. We were later met with hugs, prayers, and kind words about who he truly was. Middletown Park is people helping people. This is not just a job it is care given with heart. Thank you for making my father’s final days peaceful and for supporting our family through the hardest goodbye. If you are fortunate enough to be admitted here, count your blessings.
I love the care my relative receives at this facility but please fix the bird problem. Hundreds of birds pooping all over- I need a car wash every time I leave there!
Please take the time to do a tour of the facility and like me you will be impressed. Having had to transfer my mother from a horrible rehab and then coming to Middletown North, I cannot be more thankful. Clean, friendly, and beautiful. I feel like I no longer have to worry. Maryann, in admissions was truly amazing. She went above and beyond and my family is forever thankful.
Everyone at Middletown park has been wonderful for my mom. Nice clean friendly and most of all excellent care . The nurses, PT, OT have helped mom tremendously to get back on her feet. Thank you for your fantastic care
My husband, Shelly was admitted June 29th & left August 26th. We had no idea he was going to stay for that length of time. Due to his inability to walk & do day to day activities he needed a physical therapist as well as an occupational therapist. I give it a 5star review in every way. The patient, Shelly gives it a 4.5 star review because of the food! Everyone cheerful, friendly, and helpful every way! Because everyone is so nice & eager to help in every way. Jennifer Small, the head nurse was professional in every way as well as being a friend!
My wife was admitted May 8, 2025 and remained there until she passed away June 12th 2025. She was in unit five. I visited daily as did my family, so we got a good impression of the facility. I was very impressed by the Aids who do most of the work that realistically determines how comfortable and clean the patient is. They seemed to take pride in the level of care they provided and were pleasant and compassionate towards my wife. On some occasions she needed attention at the end of their shift and they stayed until they were finished. They were not watching the clock. That attitude says a lot about the culture of the organization. The nursing staff were also excellent but they were moved around a lot between the different floors from day to day so the patients and families didn't get to know them as well as the aids. To some extent they compensate for that by having a unit manager/nursing supervisor for each unit and they provide continuity. The doctor or a nurse practitioner were on the floor at some point during my visit almost every day. They were accompanied by the unit manager. Excellent communication with our family. The physical therapists and occupational therapists were very caring and compassionate but because my wife's ability to move was limited they couldn't accomplish very much. I did notice however that if they saw that a patient needed to be changed etc. they didn't wait for an Aid. They took care of it themselves. The quality and variety of the food they served was good, and they tried hard to meet individual needs and tastes but institutional food is institutional food. The entire place was kept clean and it should be noted that I did not detect that familiar smell common to nursing homes. Soiled sheets and garments were immediately moved to a closed location. People have personalities and so do organizations. Those who manage an organization gradually create that personality/culture. Judging by my family's experience at Unit Five those who manage Middletown Park Rehabilitation and Healthcare center should be proud of what they have accomplished.
