The Verdica score is a composite trust score from 300 to 850, computed from certification status, quality measures, and inspection reports maintained by CMS. A score of 850 places RAWLINS HOUSE HEALTH & LIVING COMMUNITY in IN in the excellent (800–850) band. The score reflects publicly available regulator data — it is not a rating from Verdica itself and does not incorporate paid reviews or advertising.
Verdica combines three weighted sub-scores drawn from CMS. Each sub-score reflects a different dimension of healthcare compliance — CMS star ratings, deficiency findings, and complaint outcomes — and is normalized against industry peers before being aggregated to the 300–850 scale.
Verdica pulls public records from CMS: certification status, quality measures, and inspection reports. Data is refreshed on rolling cycles, typically weekly to monthly depending on the source agency's own update schedule. See the "source data current" indicator on the entity page for the last successful refresh.
Common negative signals include immediate-jeopardy findings, high deficiency counts, or termination notices. Any of these appear in the underlying regulator records and pull the sub-score down; the composite reflects the weighted impact.

A longtime family friend of ours was a residence at this facility. Countless times he told me he was afraid when it was brought to a nurse's attention she just blew it off to anxiety. He was denied a bed bath. He was In the hospital when he died. I called and spoke with the supervisor regarding his things. I told them I was on my way to pack up his stuff. When I got there all his stuff was gone and thrown into a box and was in the sunroom. Less than 1 hour from the time I called until I got there and this is how he had been treated. His items look like they had been set out for trash. I would not put my dog in this facility!!! So unless you're prepared to spend a lot of time with your loved one I would recommend you find somewhere else.

I am heartbroken and honestly appalled by how this facility treated a grieving family during one of the most painful moments of their lives. A close friend of mine just lost someone she loved dearly. She clearly communicated to staff, just two hours before that she would be arriving to gather his belongings herself. This wasn’t just about items; it was about saying goodbye, about closure, about honoring someone she loved. When she arrived, everything was already packed up. That moment, one that can never be given back, was taken from her without a second thought. What made this even more painful was the complete lack of genuine empathy. The apologies felt hollow, rushed, and insincere. There was no compassion, no understanding, no acknowledgment of the emotional weight of what had just been taken from her. It became obvious that the priority was clearing the room for the next resident that was moving in less than 24 hours after his passing. This is not just poor service, this is a failure in basic human decency.Facilities like this are entrusted with people’s loved ones at their most vulnerable stages in life. That responsibility does not end the moment someone passes away. Families deserve time, respect, and compassion not to be treated like an inconvenience. No one should ever have to walk into a place expecting closure, only to leave with more pain. This facility needs serious changes in how they train staff to handle loss, grief, and communication. No family should ever be made to feel this way again.
Lack of empathy for loved ones personal items.
I am extremely disappointed with the care my family member received at this rehab facility. He was able to walk when he was admitted, but during his stay, there was little to no effort made to keep him active or continue working on his mobility. We expected physical therapy and consistent support, but that simply did not happen the way it should have. The overall level of care felt very minimal. Basic needs were not handled properly, and communication from staff was lacking. To make matters worse, the food was terrible and did not seem appropriate for someone trying to recover and regain strength. When he was discharged, he was sent home without essential items like a bed pan or proper discharge preparation. It felt rushed and unorganized, and we were left scrambling to make sure he had what he needed. I would strongly encourage anyone considering this facility to ask detailed questions about therapy frequency, discharge planning, and patient care standards before making a decision.
The difference between the “regular” halls and rehab hall is night and day. If the other halls ran as well as rehab- I would give 5 stars! The nursing staff and therapy on rehab treated my family member wonderfully as if they were their own. Glad we were able to make the switch
Rawlins House (Pendleton, IN) has proved to be an excellent place for our Mother, who has been dealing with dementia for many years. She went from independent senior living, to assisted living in Fishers, IN to memory care in Indianapolis, IN. Each place was nice, well run and she was able to have her own apartment in Fishers and a studio apartment in Indy. Her place in Indy relied on Medicaid Waivers and when the waitlist became so large and her dementia got worse; we transfered her to Rawlins House in Pendleton. We weren't sure how she would adapt to having a roommate and going to a shower room for showers. The staff on Memory Care, went out of their way to help her adapt and to make her feel welcome. She will have been there a year in February, 2026. Even though the dementia is getting worse she continues to do well in the environment that they have there. She never participated in BINGO before, but she has joined in throughout her time there. She loves to walk (ALOT) and she is able to walk around as much as she likes. During nice weather, they take her out to the courtyard, so that she can walk outside. They have therapy dogs that come to visit and performers that come to sing and play instruments. These are activities that she particularly enjoys. She can be "combative" at times and can refuse to take showers. However, the staff do a great job of getting her to shower regularly and are able to redirect her when she gets agitated. The Medical Staff have worked to change her medication and adjust dosages to help with the agitation, while being very aware of not "overmedicating" her. The staff call with any updates on her meds and share the findings of the medical staff. She can be combative with family, so some things that we might normally do are actually NOT in her best interest. The staff work with us and help with things (like, trying on new clothes) that could bring up issues and upset her. While this is not technically their "job", they have been kind enough and concerned enough about her reactions to do this for her and for us. They have regular Care Meetings, which are attended by her son and I can participate by phone. The dementia is affecting her gait and she has had a couple falls. They assessed her, contacted the Medical Staff on call, contacted us and called for an ambulance when needed. They are very helpful when we have any questions. Jessica, the Memory Care Nurse is wonderful!! She goes out of her way to help our Mother and to help us. Kelly (billing) has always been extremely helpful in answering questions. There have been 2 Coordinators since our Mother arrived, Amanda and now, Jim. Both have been very helpful. The Physical Therapy and Occupational Therapy staff have worked with her, also. They strive to help her stay active, as safe as possible and doing things that help her physically. Even though she "graduated" from therapy; they have an additional program where staff will walk with her and work to cue her to be safe, while giving her attention, interactions and the exercise she enjoys. There was an incident in the middle of night, and staff took her needs into consideration---rather than asking her to go back to bed (which can upset her) they have her go out to the common area--have a snack and walk around until she is ready to return to bed. No "power struggles" involved, just looking at what she needs and what works best for her. This attention to detail and working with her (and us) for her best interest is an amazing thing and greatly appreciated by us. We are blessed to have her in Memory Care at Rawlins House (Pendleton, IN). Congratulations to Rawlins House Senior Living, Pendleton, IN for being among News weeks 2026 top ranked Nursing Facilities in the United States and ranked No. 5 (100-149 beds) in the state of Indiana!!
The people at Rawlins House love what they do and provide amazing care for all their residents. The remodeled building looks fantastic and is very clean inside.
