The Verdica score is a composite trust score from 300 to 850, computed from certification status, quality measures, and inspection reports maintained by CMS. A score of 850 places WESTMINSTER OAKS in FL in the excellent (800–850) band. The score reflects publicly available regulator data — it is not a rating from Verdica itself and does not incorporate paid reviews or advertising.
Verdica combines three weighted sub-scores drawn from CMS. Each sub-score reflects a different dimension of healthcare compliance — CMS star ratings, deficiency findings, and complaint outcomes — and is normalized against industry peers before being aggregated to the 300–850 scale.
Verdica pulls public records from CMS: certification status, quality measures, and inspection reports. Data is refreshed on rolling cycles, typically weekly to monthly depending on the source agency's own update schedule. See the "source data current" indicator on the entity page for the last successful refresh.
Common negative signals include immediate-jeopardy findings, high deficiency counts, or termination notices. Any of these appear in the underlying regulator records and pull the sub-score down; the composite reflects the weighted impact.

Writing on behalf of my grandfather, Nathan Thompson. He was there for three weeks and was provided immaculate care. Rhonda Miller the DON and her team answered every question, made sure he was taken care of, showered. They made sure his meds got straightened out and strengthened him back enough to come home quicker than anticipated. We are so thankful for the care they provided. From admission to discharge everything was flawless and could not ask for better care. Cleanest facility we have been in in Tallahassee. Highly recommend sending your loved ones there.

Don’t bother to contact unless you can wait three years (yes 3 years). My wife contacted them for her Mom. Three year wait list for Medicaid patients. They really don’t want any Medicaid patients apparently. Not sure why they even accept it. My MIL with dementia may not even be around at that point.
They do not deliver the level of services you pay for & think your loved ones will receive. Overpriced. Clearly the staff are not well treated and this carries over into patient care. Don’t let your loved ones end up there. There are far better choices if you want quality care. Do your homework before getting involved with this place. Although I am not a "primary contact", I am a direct family member. You can see the level of their dismissiveness by their response to my review. Perfect example.Check out other review sources (Glassdoor & US News Health & many others). You'll see a pattern.
Horrific! My Aunt suffered tremendously and Westminster Oaks refuses to accept responsibility! I've already discussed it with you. You did nothing to resolve the issue.
There are not enough words to express my appreciation to Rhonda Miller and her staff. The quality of care they gave my PawPaw was exceptional even on the days where he was stubborn. My family was treated with respect, with compassion and with the willingness to help him and my family's needs was amazing. Thank you - for taking such good care of my PawPaw. Y'all are the true heros of Healthcare.
I would like to express my deepest gratitude to the staff at Westminster Health Center. My mother experienced a fall in August fracturing her ankle leaving her immobile, and from the moment of admission, the team made the entire process seamless. Cookie, who works in the front office, was the first person I met. She checked on my mother every single day and always ensured her needs were met. Her compassion and work ethic are truly unmatched. If the healthcare industry had more individuals like Mrs. Cookie, it would be a much better place. No matter how many times my family called, she remained patient, kind, and professional whether setting appointments, coordinating transportation, or ensuring staff were present to assist. Nurse Keisha, thank you for always going above and beyond for my mother. Your care and dedication did not go unnoticed. Mrs. Lavern, CNA, thank you for the tender, loving care you provided daily. You had such a tremendous impact on my mother’s healing process, and we will never forget you. If you’re searching for a rehabilitation center for your loved one, I highly recommend Westminster Health Center. The therapy team is outstanding my mother walked out the center at discharge, and the nursing staff truly embodies what compassionate care should look like! Thank you, The Monlyn Family!
Don't come here.
The entire staff is knowledgeable, caring, courteous and outgoing. From cna's, housekeeping, nurses, doctors office staff to the kitchen were outstanding. Special thanks to the physical and occupational therapist Kesley and Stephanie for their directions and instructions. Can't forget Patrick on the second floor, a good young man with a great personality and was willing to go above and beyond for you also a good conversationlist. If I ever need rehab again Westminster Oaks is my only choice. I was also impressed by the longevity of the staff, many had been there 10+ years, a remarkable feat for the healthcare industry. Thank you VERY MUCH James Earp Room 222 7/3 - 7/13/2025
