The Verdica score is a composite trust score from 300 to 850, computed from certification status, quality measures, and inspection reports maintained by CMS. A score of 850 places WESTMINSTER ST AUGUSTINE in FL in the excellent (800–850) band. The score reflects publicly available regulator data — it is not a rating from Verdica itself and does not incorporate paid reviews or advertising.
Verdica combines three weighted sub-scores drawn from CMS. Each sub-score reflects a different dimension of healthcare compliance — CMS star ratings, deficiency findings, and complaint outcomes — and is normalized against industry peers before being aggregated to the 300–850 scale.
Verdica pulls public records from CMS: certification status, quality measures, and inspection reports. Data is refreshed on rolling cycles, typically weekly to monthly depending on the source agency's own update schedule. See the "source data current" indicator on the entity page for the last successful refresh.
Common negative signals include immediate-jeopardy findings, high deficiency counts, or termination notices. Any of these appear in the underlying regulator records and pull the sub-score down; the composite reflects the weighted impact.

⭐️⭐️⭐️⭐️⭐️ Five Star Review Westminster Health Center at World Golf Village gave me my husband back. He arrived after being discharged from Baptist South in critical condition, and his 20 day rehab stay here was truly our last hope. After years of trying to get him the level of care he needed, Westminster was the first place that fully delivered. Their team was skilled, attentive, compassionate, and consistently present. They didn’t just care for him — they rescued him. He was on his way out, and they brought him back to me. The staff also went above and beyond in preparing for his discharge, arranging top rated private duty nursing and home care so he could continue receiving the support he needed. Their effectiveness and intelligence far surpassed any other rehab in the area. I will forever be grateful. Just to name a few: Nancy, Amanda, Belle, Heather, Danielle, Amy, Oksanna, Jenna, Rhonda, and Doreen. Each of them showed genuine care, and nothing was ever too small or too much trouble. They made an impossible situation survivable and gave us hope again. Westminster Health Center WGV is exceptional. They delivered when no one else could.

My husband and I chose Westminster St. Augustine because we already knew it was an exceptional community. Since my father had lived here previously, we were familiar with its outstanding independent living, as well as the assisted living and continuing care available when needed. When my husband fell and broke his hip, he was admitted to the Westminster healthcare facility where he received excellent care from a skilled medical team and compassionate staff. He has since joined me in our new residence and continues physical therapy at the health center where the staff continues to provide ongoing support through recovery and any changes in needed care. We enjoy a welcoming community of friendly residents and helpful staff, excellent food and service in the beautiful dining room, and a full range of activities and fitness options that keep us physically, socially, and spiritually engaged. There is always something happening—fitness programs, events, outings, and volunteer opportunities. I highly recommend Westminster, St. Augustine to anyone looking for a maintenance-free, active lifestyle with access to ongoing health services and care.
We love living at Westminster. The people are great and the accommodations are wonderful. We enjoy the food in the dining room as well as the cafe. We are enjoying all the activities and visiting with our neighbors. It’s a great place to live!
We were attracted to Westminster St Augustine because we knew it is nonprofit—as a 501c3, their revenue is committed to their mission. I’m writing this as I sit with my dog under a canopy of blooming jasmine, enjoying the peaceful sounds of our courtyard fountain. When you tour here, you’ll see it’s designed to bring peace: homes with tall windows so daylight lifts the spirit…well-tended gardens…shuttle drivers standing by in case you want a lift to the clubhouse or health center. Activities for everyone to keep learning, stay fit, and make social connections…factors important to longevity. I have been here just 7 weeks and the neighbors are welcoming to newcomers and quick with invitations to get involved. Plus, at Westminster St Augustine, rental fees include a monthly allowance for dining or other services. The restaurant-style menus include healthy ingredients known to support aging bodies. The waterfront campus is beautiful, with an abundance of birds to watch and paths to walk. I am glad we chose to age in place here, where we can have assistance when we need it both now and in future. Considering the number of 90th-100th birthday celebrations we’ve already seen here, I think it was a wise decision!
My husband and I are thrilled that we will shortly be residents at Westminster St. Augustine, located in World Golf Village, St. Augustine, FL. Over the past 8 years searching for a continuum of care environment, we have found it to be a very friendly, active community with a very helpful and compassionate staff. Residents can enjoy many social, educational, and fitness opportunities, while living in spacious, independent living homes, before progressing to assisted living and skilled nursing. It gives us much comfort to be part of a wonderful environment with continuum of care!
We moved to Westminster nearly 6 years ago and it has turned out to be a great move. Lots of new friends and activities that keep us busy. Everyone is extremely friendly. You can walk down the street and everyone will wave---residents, maintenance personnel, or housekeeping personnel.
There is so much to do here! I can't get over all the different performances, classes, and outings.
We visited Westminster St. Augustine seven years ago and were impressed with the beautiful setting, living accommodations, friendly staff, and programs offered. We moved to Westminster six months ago and are so happy to be in this beautiful, vibrant community. There are so many activities to engage the body, mind, and spirit! Our new home in Southpointe is lovely and our neighbors are a blessing. The Team Members have been so helpful and caring and have made every effort to make our transition as seamless as possible. Residents are friendly and welcoming. This feels like "home" now as we are making new friends and engaging in activities.
